| Nomination Process |
| STEP 1. You must currently be a MICA member in good standing to submit an award nomination. If you are not a member and would like to join or renew your membership please go to the MICA MEMBERSHIP page. STEP 2. These are once in a lifetime awards. To see if an individual has already received the award you are nominating them for use the AWARD SEARCH tool or check the AWARD RECIPIENTS ALPHABETICALLY. STEP 3. Download the nomination form either as a WORD doc or a PDF file. NOMINATION FORM - WORD doc NOMINATION FOR - PDF file (Print and complete.) Complete the nomination form and Email to execdir@micorps.org. In the event you are making multiple nominations simply complete the nomination form multiple times. STEP 4. A $40 payment is required for each nomination. Payment is made online by Visa, MasterCard, American Express or Discover. Credit Card details are collected using a SSL secured server. For multiple nominations change the quantity in the Shopping Cart and RECALCULATE. Once you have completed and Emailed the nomination form(s) click this link and PAY NOW. STEP 5. The MICA National Chapter will mail the certificate and medal to the shipping address specified on the nomination form. Please do not hesitate to contact the Executive Director at execdir@micorps.org for any additional information or assistance. STEP 6. Please avail yourselves of the following sample materials for your Award Presentation Ceremony: You can download the following DOC files. KNOWLTON AWARD CEREMONY CANDLELIGHT CEREMONY GOLDEN ROSE AWARD CEREMONY THOMAS KNOWLTON. (We include a biography of him for your printed program.) Step 7. Many nominators and presenters have purchased MICA Memberships for their Awardees, and we can provide a certificate of that membership for your presentation. This is a very nice gift. There is no automatic MICA Membership associated with receiving an award. Click MEMBERSHIP to purchase a MICA Membership for your Awardees. |
